Licensed Practical Nurses
Licensed Practical Nurses may be assigned leadership tasks in alignment with the organization’s policies and applicable laws. This may include participating in staff hiring, training new team members, coordinating daily assignments, evaluating job performance, offering feedback or disciplinary actions when needed, and helping to resolve workplace concerns. Unit Managers maintain overall responsibility for the supervision and effectiveness of their respective units.
Key Duties and Responsibilities:
- Assess, plan, and evaluate nursing care to ensure alignment with each resident’s individualized care plan.
- Collaborate with the interdisciplinary team in discharge planning to support seamless care transitions.
- Assist with interviewing potential staff and participate in the onboarding process.
- Prepare and submit timely performance evaluations for assigned team members.
- Address employee performance concerns with coaching or corrective actions as needed.
- Administer prescribed medications to residents in compliance with current regulations.
- Deliver wound care and treatments according to established protocols.
- Maintain accurate, up-to-date documentation across all required facility records.
- Perform comprehensive head-to-toe clinical assessments on residents.
- Help identify early signs of health concerns to reduce unnecessary hospital transfers.
- Accept assignments from supervisors with a positive attitude and a sense of accountability.
- Carry out other related tasks or responsibilities as assigned to support resident care and team operations.
Certified Nursing Assistant (CNA)
Certified Nursing Assistants (CNAs) provide essential support to clients by assisting with personal care, health-related tasks, light housekeeping, and activities of daily living. They are responsible for observing, documenting, and reporting any changes in a client’s condition to the supervising nurse. CNAs must demonstrate proficiency in all required skills and perform only those tasks included in the client’s individualized care plan.
Key Responsibilities:
- Assist with personal hygiene tasks such as bathing, dressing, grooming, toileting, and incontinence care, including the use of bedpans, commodes, or toilets.
- Support client mobility by aiding with transfers, repositioning, and ambulation, using assistive devices like canes, walkers, or wheelchairs as needed.
- Provide reminders for medication intake as outlined in the care plan.
- Perform light housekeeping duties including cleaning, laundry, and home organization to maintain a safe and comfortable environment.
- Prepare and serve basic, modified meals according to dietary instructions and assist with feeding when necessary.
- Deliver routine skin care and assist in applying over-the-counter skin lotions as directed.
- Measure and record vital signs (such as temperature, pulse, and blood pressure), if included in the care plan.
- Observe and promptly report any changes in the client’s physical or emotional condition to the supervising nurse or care manager.
Certified Medication Technician (CMT)
The Certified Medication Technician (CMT) is responsible for safely administering prescribed medications to residents or clients under the supervision of a licensed nurse. The CMT also provides basic direct care services and assists with daily living activities as needed. The role requires attention to detail, strong communication skills, and a commitment to providing high-quality, compassionate care.
Key Responsibilities:
- Administer medications accurately and in accordance with physician orders, facility policies, and state regulations.
- Observe and report any adverse reactions or changes in resident condition to the supervising nurse.
- Document medication administration and related observations promptly and correctly in the medical record.
- Assist residents with basic personal care tasks, including grooming, dressing, hygiene, and mobility support.
- Monitor and record vital signs when required.
- Maintain a clean, safe, and organized medication cart and storage area.
- Ensure proper infection control practices are followed during medication preparation and administration.
- Maintain confidentiality and residents’ rights in all interactions and documentation.
- Participate in in-service training, staff meetings, and continuing education programs as required.
Geriatric Nursing Assistant (GNA)
The Geriatric Nursing Assistant (GNA) provides direct patient care and assistance with daily living activities to elderly residents or clients under the supervision of a licensed nurse. GNAs play a vital role in promoting comfort, dignity, and safety for patients in long-term care, assisted living, or home health settings.
Key Responsibilities:
- Assist residents with personal care tasks such as bathing, dressing, grooming, toileting, and oral hygiene.

Home Health Aide (HHA)
The Home Health Aide (HHA) provides essential personal care and supportive services to clients in their homes under the direction of a licensed nurse or healthcare professional. HHAs assist clients with activities of daily living (ADLs), ensure their comfort and safety, and promote independence in a compassionate and respectful manner.
Key Responsibilities:
- Assist clients with personal care tasks including bathing, grooming, dressing, toileting, and oral hygiene.
- Support clients with ambulation, transfers, and mobility using appropriate techniques and equipment.
- Provide assistance with meals, including preparation, feeding, and monitoring dietary restrictions.
- Perform light housekeeping duties such as laundry, bed making, and maintaining a clean and safe living environment.
- Offer medication reminders according to the care plan.

Registered Nurse (RN)
The Registered Nurse (RN) is responsible for providing direct and comprehensive nursing care to patients in accordance with physician orders, established policies, and state nursing regulations. RNs assess patient health needs, develop and implement individualized care plans, monitor progress, and collaborate with the interdisciplinary care team to ensure the delivery of safe, high-quality care.
Key Responsibilities:
- Perform comprehensive assessments of patients’ physical, emotional, and psychosocial needs.
- Develop, implement, and evaluate individualized care plans based on patient needs and clinical judgment.
- Administer medications and treatments accurately and in accordance with physician orders and protocols.
- Monitor patients for changes in condition, respond appropriately to emergencies, and notify physicians or supervisors as needed.
- Provide patient and family education regarding health conditions, medications, and care strategies.
- Coordinate and collaborate with healthcare professionals, caregivers, and support staff to optimize patient outcomes.
- Document all assessments, interventions, and patient responses in a timely and accurate manner.
- Maintain compliance with infection control, safety, and quality standards.
- Supervise and delegate tasks to Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and other support staff as appropriate.
- Participate in training, staff development, and performance improvement initiatives.
